Dentistry on the Ponds Savings Plan is a dental discount plan and is NOT dental insurance. It cannot be combined with any other dental insurance. The savings plan will not apply to dental care costs for injuries covered under medical care disability or workman’s compensation type insurance. The savings plan will not apply to cosmetic services or Orthodontic procedures. The savings plan cannot be combined with any other offers or discounts. An annual fee is required at the time of enrollment and is non-refundable and non-transferable. No refunds are given if the patient chooses not to use their dental plan.

The Savings plan expires one year from the date of purchase. Plan participant is responsible for utilizing their benefits within one year time period. Services not utilized at the end of the membership period are not carried over to the following year. Plan participants are responsible for scheduling their periodic treatments. Discounts under this plan shall not apply to any treatment started prior to enrollment or after the expiration date. All non-preventative services, including diagnostic X-rays, will receive a 15% discount. All Payments are due on the day of service to receive the discount. Any services that are not paid for at the time of service will be billed at the usual higher fee.

Dentistry on the Ponds reserves the right to modify, change, or discontinue the Savings Plan fee terms and services at your company’s discretion upon written notice from Dentistry on the Ponds prior to your anniversary renewal date.
